Abstract

Occupational therapy (OT) has demonstrated effectiveness in supporting individuals undergoing hemodialysis, a life‑altering yet lifesaving treatment for end‑stage renal disease (ESRD) and chronic kidney disease (CKD). Despite this, hospitalized patients who require dialysis often receive reduced rehabilitation services due to the time‑intensive nature of dialysis and the productivity demands placed on therapists. Additionally, gaps in therapist knowledge regarding the unique functional deficits and educational needs of this population further limit service delivery.

This doctoral capstone project (DCE), conducted at Upper Peninsula Health System (UPHS), a hospital in rural Northern Michigan, addressed these barriers through the development of a comprehensive resource “toolkit” designed to equip therapists and patients with practical, evidence‑informed resources. Working directly with therapists and patients allowed for meaningful, context‑specific program development tailored to the needs of this underserved population.

Alongside resource creation, the project advocated for and initiated intradialytic occupational therapy (IDOT) sessions. Through collaboration with key stakeholders, education on the benefits of intradialytic intervention, and supervised implementation with select patients, the project expanded opportunities for therapy engagement and enhanced clinical practice skills. The significant daily functional challenges faced by individuals on hemodialysis fall squarely within the scope of OT, and this project contributes to the profession by promoting service equity and establishing a framework for IDOT integration.
