Abstract
This paper is a detailed report of a system created for Gazelle Sports. In this project I have created a data-entry system to cut down on the time spent to enter the surveys and several reporting functions to accurately portray the results of the surveys. Previous time spent to enter surveys was about twenty minutes per survey, and is now about two minutes per survey. The reporting functions for the results can now be viewed in many formats, focusing on several different part of the survey. I have outlined my activities for this project by giving a background of both Perspectives Consulting and Gazelle Sports, the proposal for this project, diagrams to conceptualize the flow of the project, a data dictionary to detail the specific fields used, and copies of the reports that are generated.
