Abstract
Health Information Technology (HIT) working in conjunction with the long term care facilities is vital to improving care. Currently, HIT has not been optimized and duplicate documentation is completed, causing more work for the staff. The important aspect of HIT is getting the certified nursing assistants, dietitian, care coordinators, managers and registered nurses and the entire clinical care team to come together to discuss what information is important and how they would like to input that information.
